This one "smart" card would combine a drivers license, multiple credit cards, club cards (such as AAA or Costco), medical benefits cards and etc. The card itself would be a hybrid of existing id cards that have a photo, birthdate, address, signature, and magnetic strip (or chip) along with a lcd display and input key. The magnetic strip/chip would contain information (i.e. institution name, account number) for each account or membership represented. The small lcd display would be similar to that found on a SecureID (http://www.rsasecurity.com/node.asp?id=1158) and would be used to display information from the different accounts. The input key would can scroll through the different accounts.

And then have the thing encoded into a RFID shoved under your skin on your forehead and you're all set. Of course it must have an unbreakable code keyed to your DNA or eye pattern or fingerprint to make it safe. Joined with a global positioning indicator you can become a member of the human ant's nest available to government offices around the world at their slightest behest.
It has been brought up here before. I don't like it because one organisation could bring up information from another.
I suppose you could have just one big number that an organisation uses how they see fit to recognize you, or recognize in their own proprietary way. The downside of that is theives could steal your whole number.
Would be even better to have a smart chip implanted. Think about women willing to have a inch long contraceptive implanted with a needle that needs to be replaced evey couple of years!
