When there is an accident and the person involved cannot communicate, who do you call with his/her phone? You might try the last dialed number and it may result in giving someone a heart attack.

My suggestion is simple: As we have the number 1 as the official voice mail speed dial, WHY NOT have another, like the number 2 that will be for emergencies? So when next you come across an accident, you know just whom to call.

What's wrong with 911?

On at least one of my cell phones, holding down the 9 key would dial 911. I don't know if that was specific to that make phone, or somebody programmed it in though.

This is intended universally. 911 does not exist in all countries. Besides, if the key is universally defined, people can choose the number they want to program. 911, 112, their country's emergency number, family doctor, mother e.t c.

I thought it was universally known, but obviously not well enough. This idea is already covered by an ICE contact "in case of emergency," which is defaulted as a contact in many phones. I have heard of the ICE contact being automatic speed dials as well.

I think I've heard of the ICE but never seen it. an ICE contact in the phone will mean the possibility of having one number under two names. (My dad might be my ICE contact). Most phones do not support this. I think that is why it has not caught on. An official key would have it's own symbol which will be understood irrespective of the language barrier.
