I suspect this is already in the works if not already available, but I'll post it anyway. Create a completely hands-free phone that is completely voice activated. There would no buttons at all except maybe an on/off switch. It could use either a headset or a small earpiece and a clip-on microphone (maybe bluetooth) that attaches to your collar. The main part of the phone (with the batteries and transceiver) could stay safely in your pocket, purse or belt holster. You turn it on by pressing a button on the earpiece or microphone. Once on, you can call numbers by simply saying the number out loud into the mic. You could also program numbers you call regularly the same way you can on many cellphones already. All the menu options could be selected and used by saying the appropriate words. If you forget the names of the menu items, you can just ask the phone to list the menu and it will read off all the menu items to you. The advantages of this include: You wouldn't have to use you hands at all except to hit the on button. The device could be made as small as the electronics will allow. Current phones are limited in their miniaturization by the need to fit the keypad. Simplification of operation. You just talk to the phone and it answers you. Eliminates redundant input/output devices. A phone has to have a speaker and microphone anyway. Having a keypad and display is unnecessary if the speaker and mic can suffice for all the input/output needs.

You could even make microphones that look like the communicator pins in Star Trek.
i think the idea of this option on a phone is great, i have thought many times while using my headset that i wish i could change a setting, or recall the number of a recent call the way i can with buttons, but i think that the buttons are still necessary, as much as i would like to eliminate them and shrink the phone even more, sometimes the buttons are necessary, i know that my phone is not accurate enough in voice recognition to perform basic functions without the need for confirmation, and especially in a noisy environment the feature becomes useless, a human can distinguish your voice just fine once the call is established, but the phone is completely unable to perform.
Yes, the voice recognition software will have to improve, but this can happen and no doubt will. You mention the need for confirmation. If the device is unsure what you said, it could ask you "did you say...?" and you could tell it yes or no.
